By submitting this consultation form, I affirm and certify that:
- I am of sound mind and at least 21 years of age.
- I am permitted by law in my locale to receive the medication(s) I
am requesting for my personal use only.
- I have recently undergone a complete physical examination and medical
history evaluation by my own primary care physician who is available
and whom I agree to contact for any necessary local follow-up care and
intervention.
- I have been fully informed by health care personnel and have studied
written or internet materials on these drugs and understand the risks,
benefits, and possible side effects of the prescription drug(s) I may
request.
- I have previously used the medication(s) I may request safely, while
under a physician's supervision, and/or I been advised by my examining
physician that the use of the medication(s) is not contraindicated for
me and is appropriate for my personal therapeutic and medical needs.
- The medication I am requesting is solely for my own personal therapeutic
and medical needs, and I agree not to distribute any of the medication(s)
to others.
- When reviewing my request, I am requesting that a U.S. licensed prescriber
act only in an adjunct capacity to my local physician, and not replace
my local physician. I further request the prescriber to authorize the
prescription drug(s) for dispensing by the clinic's associated licensed
pharmacy.
- I am seeking the requested prescription(s) to have a necessary supply
of medication on hand, and will not stock this medication beyond an
adequate supply.
- I will promptly contact a local physician for any necessary medical
intervention should a complication or concern result related to the
use of a requested medication.
- I will not take any over-the-counter medicines without full disclosure
to my pharmacist and primary care doctor of all medications that I am
taking.
- I am authorized by law to use the credit card that will be used if
my request is approved and processed.
- I have answered and will answer all questions truthfully, for my safety,
just as I would in my local physician's office and under that physician's
care. I have fully and completely disclosed any and all information
concerning my health and medical history that may possibly be relevant
to my request for this medication.
- I realize there are risks as well as benefits to any medication, even
OTC drugs. As such, I have been fully informed of the effects, risks,
and benefits of this medication. I consent to treatment as I may request.

WAIVER AND CONSENT AGREEMENT
I hereby release NextDaySoma.com and all of its employees and contractors
including physicians from any and all liability whatsoever associated or connected
with my consultation and/or the use of any prescribed medications. I hereby
state that I am an adult and that I am aware of the potential side effects associated
with prescribed medications including pulmonary hypertension, heart failure,
or even death.
I understand that no doctor, nurse, or administrative personnel can guarantee
that medications, even if prescribed, will provide the results I seek. Further,
I understand that even if prescribed, I may suffer adverse effects from these
medications. I hereby release NextDaySoma.com and all its employees
and contractors including physicians from any and all liability whatsoever associated
with any adverse effects I may suffer from the use of the prescribed medication.
I am participating in this program at my own choice, at my own expense and my
own liability and assume all responsibility for my use of the prescribed medication.I
fully understand that it is my responsibility to have an annual physical examination,
including any suggested laboratory test, to ensure that I have no disease(s)
that might make the prescribed medication inapropriate for my condition. I further
agree that I have consulted with my physician and/or pharmacist and hereby warrant
that I am not taking any other medications that are contradicting to the medication(s)
being prescribed. I further agree to immediately notify any doctor whose present
care I am under that I have chosen to take this medication so that they may
advise to continue or discontiune use.
I understand that NextDaySoma.com is not a pharmacy, but an intermediary
with a network of licensed pharmacies and physicians throughout the United States.
Please note that our U.S. Licensed Physician requires that all Pain, Sleep/Stress/Anxiety,
Muscle Relaxers, and Migraine Headache medications include a Doctor Consultation.
Also note that all medications are for the intended use of the individual to
whom it is prescribed and is not distribution or resale of any kind.
We are unable to accept returns or issue refunds for any orders due to the fact
that these are prescription medications.
Customer is responsible for all customs, tariffs, and taxes applicable to their
